Carers

Medway Council and NHS Kent and Medway recognise the essential contribution that carers make. We support Medway carers of all ages, to continue to carry out this valuable role and support your health and wellbeing.
Carers First has been funded to:
- provide advice, information and guidance in your caring role
- support you in accessing carers respite or support payments that can help you in a crisis, for those eligible
- develop contingency and emergency planning
- provide access to immediate practical or financial support
- offer signposting for a Statutory Carers Assessment.

Did you know patients can self-refer to physiotherapy?
Patients can complete an online or paper referral form outlining why they feel they need physiotherapy, which will be returned to the musculoskeletal (MSK) triage service for screening by a senior clinician. If physiotherapy is the appropriate treatment, the patient will be able to choose from all AQP providers in Medway or Swale and the referral will be sent to that provider to book an appointment.
If the referral is deemed suitable for the orthopaedic clinical assessment service (CAS) and not physiotherapy, these referrals will be accepted by MCH and an appointment will be booked when the patient reaches the top of the CAS waiting list. We will also accept patients returning for repeat injections or follow ups when they have received care through CAS previously.

Kent and Medway Wellbeing Hub for Health & Care Staff
The Kent and Medway Wellbeing Hub has been designed for coleagues working in the health and care sectors.
It is important to prioritise all aspects of our own health and wellbeing, as well as looking out for each other. On their website you will find links to a wealth of advice and services for when you need a little extra support, as well as lots of tips and resources for being proactive, helping you to stay happy and healthy.
Designed to complement existing intranets, you’ll also find information specific to your own organisation on this site.

The Glebe Family Practice has remained open throughout the COVID pandemic providing a streamlined service using digital media to support consultations and diagnosis. Quite a feat given that we suffered staff shortages due to illness and self isolation and ever increasing demand. And have simultaneously delivered 1000’s of COVID vaccinations while still maintaining core services, thanks to our wonderful nursing and support staff. A special mention goes to Helen a retired nurse who has volunteered providing essential documentation for patients, cleaning between clinics and being on hand.

The Glebe Family Practice is part of Gillingham South Primary Care Network, a group of practices in the locality serving 80 000 patients in total. Funding via the PCN has expanded the workforce in the the PCN. Thanks to the PCN we now have the services of a Clinical Pharmacist, First Contact Physio and a Social Prescriber. These team members are shared across the network, and there are plans to expand these resources to include paramedic and mental health practitioners in the forthcoming year. It is hoped this will go some way to addressing the shortage of trained GPs.

Online Services
If you have registered for Patient Access online patients services, you can use the website or app to request and cancel appointments, update your personal details and view your medical record online.
Remote Consultations
Our new remote triage & consultation system, Anima, allows you to fill out a simple online form to request medical advice and appointments and make administrative requests such as sick certificates and medical reports.
