We have an open list and welcome requests for registration from patients living in or moving into the practice catchment area.
If you live within our practice catchment area, please collect a patient registration form on the designated day and return it to us with photographic proof of your ID, eg your passport or driving license and proof of address, eg a utility bill or bank statement with your name and current address.It would also be useful to have details of any repeat medications you are being prescribed by your current GP.
Change of personal details
If you change your name, address, telephone number or email, please remember to inform us. This may be done by visiting the surgery, phoning us, or by updating your record at Patient Access (you need to be a registered Patient Access user).
Practice List and Out Of Area Deregistration
The Glebe Family Practice maintains a list size of around 5000 patients which is above the national average for the number of fulltime GPs. We have expanded the staff to include a pharmacist and 2 Advanced Nurse Practitioners a Practice Nurse and a Healthcare Assistant. Everyone at the Glebe works hard to ensure that a high standard of care is provided to all our patients. We have an ongoing obligation to the local community to keep the practice list open and previous attempts to close the list have been declined by the Primary Care Team (PCT) at Kent and Medway Clinical Commissioning Group (KMCCG) as recently at November 2021. Historically the practice boundary was a lot larger and we have kept patients on who have not moved, although the PCT at KMCCG indicated that they would support wholesale removal of this group to manage pressures, something that we would not countenance.
We have a statutory duty to our staff to prevent burn out and to our existing patients to avoid errors and maintain high standards of care. We do, therefore, have to manage the list size which means limiting the number of new registrations to match the number of patients leaving the list and asking patients who move out of area to register elsewhere.
We do appreciate that this can be upsetting especially when patients are not far from the boundary and have had a long history with the practice. We don’t make exceptions to the rule as this would be inherently discriminatory.
We hope that this explanation will go some way to explaining the situation with regard to Out of Area De-registrations and reduce the volume of correspondence on the subject. Your understanding is much appreciated.
Non-registered patients
An appointment for immediately necessary treatment is only available to people who have incurred an accident or medical emergency at any place in our practice area and who require GP services. If the resulting condition is potentially life threatening an ambulance should be called using the 999 service.
In addition, if you are away from your normal place of residence and are staying within our practice area for more than 24 hours and less than 3 months, you may also request an appointment.

Online Services
If you have registered for Patient Access online patients services, you can use the website or app to request and cancel appointments, update your personal details and view your medical record online.
Remote Consultations
Our remote triage & consultation system, eConsult, allows you to fill out a simple online form to request medical advice and appointments and make administrative requests such as sick certificates and medical reports.
